Before My Surgery

Thank you for choosing the Charlotte Surgery Center.  We are honored to provide your care during your surgery or procedure.  To assist us in ensuring that you have an excellent experience please carefully review the information provided here.  It is our goal to provide you with all the necessary instructions to alleviate  anxiety, provide for your comfort and reduce the possibility of complications or delays.

  • To expedite your check-in process please click the registration link at the bottom of the home page and complete the Medical One Passport (health history).  While you may have already provided this information to your doctor, it is important that our team has an opportunity to review your history for your safety. Please complete this at least 48 hours prior to your scheduled procedure date/time.
  • A Registered Nurse will contact you approximately 24-48 hours prior to your surgery, at your number of record,  to review the specifics of the information you provided during your online registration.  The Pre-Admission Nurse will also answer any questions you may have and give you an estimated time for your surgery and arrival.
  • If you have not received a call by the day before your surgery or have additional questions please call us at 704-887-3569 or 704-377-1647 between the hours of 8:00 am – 4:30 pm and leave a message for a member of our Pre-Admission team.
  • We will make every effort to adhere to your scheduled appointment times; however, because of the nature of the care we provide, unforeseen delays may occur. Our physicians give each patient the individualized care that they need during their surgery.
  • If the patient is a minor, it will be necessary for a parent or legal guardian to be present to sign the consent for surgery.  Proof of identity and guardianship is required at time of check-in.
  • It is important and a policy of our center that all patients must be accompanied by a driver/responsible party at the time of check-in.  We will ask this person to provide a cellphone number so that they can be easily reached.  Your driver/responsible party must be prepared to remain at the center for the entire duration of your procedure.
  • Public transportation such as a taxi, bus or other services like Uber are only acceptable when accompanied by a responsible adult.
  • Please understand that if you do not have a driver, at the time of check-in, who will stay for the duration of your procedure, it will be necessary for you to reschedule your surgery for another day.
